Payment- Bring this Form to Class or to Open House!

• Half due at class sign up, and second half due no later than first day of class.

• NO Refunds for any reason. (Deferment for future classes available for those unable to attend due to documented medical or family tragedy reasons.)

• Payment made to Cathe B. Jones via Cash or Money Order;
  

  • Check Policy: In-state checks accepted, with proof of Identification, including address;
• Bounced Checks result in a $45 returned check fee. If more than one check is returned then student may not register for classes in the future except by advanced cash payment only.

  Signature on this registration indicates that the student is completely aware of the payment policy, discount policy, and the NO REFUNDS policy.

• NO Student under 16 may register without expressed written or oral consent made to Cathe Jones by parent or guardian. If emancipated, student must show proof of emancipation, (wedding decree, or court order).

One (1) Class: $350
Two (2) Classes: $700
Three(3) Classes: Discounted 10% $1050 - 105= $945
Four (4) Classes: Discounted 20% $1400 - 280= $1120

Final performances-

• Each student understands that the final performance is held during the week FOLLOWING the last day of classes for that session, and student is required to bring his or her OWN blank media, (Mini DVD-R), in order to receive copies of the final performance.
  
  
• Additional DVD copies can be purchased for a nominal charge.
  
  
• Final performance is paid, $10 at the door, and consists of several classes, including but not limited to those attended by students; (ie. Improvisation 101 students may appear the same evening as Stand-up students);
  
  
• Previous students and special guests may appear as time permits.
  
  
• All performing students will receive a nominal portion of door collections as reward for their efforts, split with Comedy Clinic, and the rental of the cabaret space for that event.

Private Coaching-

• If student requests private coaching from any instructor this is done at the sole discretion of the instructor and he or she may set fees accordingly.
• Coaching MUST be done at the Clinic location, or at a predetermined public location, and may be recorded for training purposes.
• All students receiving private coaching are eligible for final performances, depending up on the instructor's opinion of the skills at the end of these sessions.
• No coaching will be held in private residence, nor without the expressed written or oral consent of Cathe B. Jones to ensure student and instructor safety and record of class history.

BEFORE REGISTRATION IS COMPLETED-

• I have read all of the above stated Registration information, and understand the discounts and class payment requirements. I also understand that I do not hold Family Music of West Sahara nor the Comedy Clinic liable for any injury I may obtain as a result of physical or mental activity.
  
  
• I further understand that after the classes have finished a performance is scheduled for all students, including those who are not necessarily registered for the same classes as I am, and it is up to me to perform or not in this last event. If I elect not to perform, I will notify my instructor within 48 hours of the final performance schedule so he may give my spot to another entertainer.
  
  
• I understand that the classes scheduled may change, and my instructor will notify me of any changes which affect the course schedule I have registered for as soon as possible using the information listed above.
  
• This has been signed without any pressure from members of Family Music, nor the Comedy Clinic staff, instructors, or assistants.
